What kind of table file did everyone submit? Should it be a picture (e.g. 
JPEG), something like an Excel file, an html, or just plain text with 
something like this:

The last, text option seems like the worst to do but I wasn't sure what 
Appendix A meant by, "in clear readable format (Appendix A, ver 1.9, Sec 
1.3.A)." 

Also, what does: "... with technical explanations of the test procedures 
and a simple summary of failures in the cover page (Appendix A, ver 1.9, 
Sec 1.3.A)" mean?  What's an example of a "technical explanation?"
