I vote with Brett--a lot has changed in the requirements since ST 1.4.5, so a retest would be good. The issue for you would be cost, I guess.
The FAQ has some guidance, though:

But if you're going to re-test, you'll have to use version 4.0.5, that's pretty much a given. But my advice is worth about as much as you paid for it. :-) The committee has final say.

You should test with 4.0.5.
FWIW, if your product was certified using Self Test 1.4.5, I would
strongly suggest re-cert whether it is required or not.
